West Mountain Radio Terms: This offer valid only with the purchase of a West Mountain Radio branded product with an actual purchase price of $50 or more. The product must be a retail purchase directly from West Mountain Radio or an authorized dealer in the United States between Dec. 8, 2008 and Dec. 31, 2008. To receive this rebate, simply fill out this rebate form. Mail the rebate form completely filled out with a copy the original purchase receipt. All claims must be postmarked no later than Jan. 15, 2009 and received on or before Jan 31, 2009. West Mountain Radio will not honor delayed submissions due to the product being out of stock. Proof of purchase required (photocopies accepted). Submissions received incomplete or without proper documentation will not be eligible. All requests and submitted materials become the property of West Mountain Radio and will not be returned. West Mountain Radio is not responsible for late, lost, damaged, misdirected, incorrect, incomplete, illegible or postage due requests, or requests that are not properly submitted for any reason.
